Chiari-ähnliche Malformation/Syringomyelie (CMSM)

 

 

Die Chiari-ähnliche Malformation und die mit ihr häufig assoziierte Syringohydromyelie wird heute weltweit bei Cavalier King Charles Spaniels und anderen kleinen Hunderassen diagnostiziert und zahlreiche Publikationen charakterisieren diese Erkrankung. Obwohl es immer noch Unklarheiten über die Pathogenese und die genetische Determination der Verlängerung der kaudalen Kleinhirnanteile gibt, wurden in den letzten Jahren Fortschritte in dem Verständnis über die Entstehung der medullären Zentralkanalserweiterung und der Progression der klinischen Zeichen erzielt. Leider gibt es immer noch unterschiedliche Meinungen zur medikamentösen und chirurgischen Therapie, doch die schrittweise Etablierung einiger Zuchtlinien mit seltener betroffenen Hunden durch konsequente MRT-gestützte Zuchtuntersuchungen scheint der einzige zukunftsweisende Weg zu sein.

 

 

Summary

 

The Chiari-like malformation and the often associated syringohydromyelia have been diagnosed world-wide in Cavalier King Charles Spaniels and other small dog breeds and latest developments have been regular published. There are still controversial opinions about the pathogenesis of the elongation of the caudal part of the cerebellum and its genetic determination. The understanding of the formation of the enlargement of the medullar central canal and the clinical signs hasmuch improved over the last years, but there is still a diversity of therapies which include drugs and surgical solutions. In the near future, the consequent employment of a MRI-based breeding scheme seems the only way to establish certain breeding lines of less affected dogs.
